Kindergarten Health Information 2020-21 School Year
    Before you know it, the 2020-2021 school year will be here and your son/daughter will be starting kindergarten! Below is a list of health requirements for kindergarten students. Forms for each requirement will be sent home in the next few weeks. The Elementary Building’s  fax # is 515-386-3483. Providers can fax already completed forms to the school.
    1.    Physical - Any physical form is accepted and is good for one year before the first day of school. If your child has had a yearly physical already completed, please call your provider and have them fax a copy to us.
    2.    Immunizations - Please have all immunizations completed before the first day of kindergarten. If you are unsure if your child needs any immunization updates, please call the elementary and they are able to look it up for you.
    3.    Dental - A dental screening is required for kindergarten. This can be completed by a dentist (preferred), dental hygienist, physician, physician-assistant, or nurse.
    4.    Vision - A vision screening is required for kindergarten. A screening will be done at school if it is not done with an optometrist/ophthalmologist.
    5.    Lead Screen - A lead screen is required for kindergarten and is typically done at your provider’s office around the one-year-old check-up. Check with your provider to be sure your student has had this done.
